Please keep your ARF Certificate of Registration in a safe place and avoid unnecessary expense due to its loss. If you should lose your certificate, we will require that you provide us your animal's "ARF Registration Number or "Litter Registration Numbers", as shown on animal's pedigree.
Note 1 - If you locate any of the above "ARF Numbers", we can issue a "DUPLICATE" ARF Certificate of Registration in the owner's name.
Note 2 - If you cannot provide an "ARF Registration Number" or "ARF Litter
Registration Numbers", please complete this application by filling in each line.
Upon completion, we can issue a "Duplicate" ARF Certificate of Registration in
the owner's name.
NOTE: If you cannot complete the required information on this application, you
can re-start your animal's pedigree by requesting an APPLICATION FOR MERIT
PEDIGREE.
